There’s an easier way to manage your family’s care: MyBSWHealth

Did you know that you can manage your whole family’s healthcare all in one place? MyBSWHealth makes it easy to keep track of appointments, prescriptions, bills and more for every member of the family—from young children to elderly parents.

It’s called proxy access, and here’s how it works.

How do I set up proxy access?

Proxy access to medical information can be established one of two ways:

1. An adult may grant or request access to another adult in their family by completing the online proxy management form
2. An adult may request access to a minor in their family by completing the online proxy management request form

Please note: this feature is only available for existing Baylor Scott & White patients. Alternatively, you can complete a Proxy Consent Form and mail it to the address printed on the form or return it to one of our medical facilities to request access:

What can I do once I have proxy access?

Once you are set up as a proxy for your loved one, you will be able to access their health and pharmacy information from within your own MyBSWHealth account. You can view your proxy’s upcoming appointments and information on the dashboard by selecting their name in the top righthand corner of the screen where your initials are displayed in a circle.

You will also be able to schedule appointments and conduct an eVisit on behalf of your proxy. When scheduling an appointment or doing an eVisit, you will be prompted to select the proxy the visit is for before the appointment is scheduled.  

With proxy access, you can seamlessly manage care needs for the whole family, all in one place and all conveniently on your favorite mobile device.

Who can get proxy access of a minor?

You do not have to be a parent of a minor to be granted access to their medical information. If you are the stepparent, grandparent or other guardian to the minor, you will be able to request access to the minor’s medical records through MyBSWHealth. You will need to upload a verification document such as a power of attorney, letter of guardianship, custody document or the physical form signed by the biological parent before submitting the request. 

How long does it take to get access?

If granting another user access to your records: Once the online form for granting access has been submitted, an email is sent to the adult you’ve granted access to accept or decline. Once they accept your invite, they will immediately be able to view your medical information or pharmacy records in their MyBSWHealth account.

If requesting access to another user’s records: Once the online form for requesting access has been submitted, an email is sent to the adult you’ve requested access to approve or decline. Once they approve your request, you will immediately be able to view their medical information or pharmacy records in your MyBSWHealth account.

If requesting access to a minor account: Once the online form for requesting access is submitted, it is immediately sent to the Health Information Management (HIM) team for review. HIM will review requests within two hours Monday through Friday, 8 a.m.-5 p.m. CST; requests received outside that window will be reviewed during the next available period.

If mailing or faxing a physical form: HIM will review requests within 5-7 business days.
