Your gift will be put to work immediately to support the community efforts and help provide much needed services to people at risk for developing chronic conditions.

Community outreach

In fulfillment of our mission, our efforts go well beyond the doors of our facility. In addition to services provided at our center, community health workers and wellness staff go out in the community to educate families and individuals about healthy lifestyles and available resources.

Research program

Our research staff leads community-based participatory research studies that evaluate new ideas to improve the health of members and patients. Our Health Innovation Partners (HIP) team is bilingual and readily helps community members get involved in learning more about their health. Please email us for more information about our current research studies.
