Jake Schlemmer, MD is a board-certified internist providing comprehensive primary care with expertise in managing chronic conditions. He utilizes a patient-centered approach by listening to patients’ concerns and collaborating with ... patients to form individualized plans of care.


Dr. Schlemmer earned his medical degree from McGovern Medical School at UT Health Houston in the world-renowned Texas Medical Center. He then completed post-graduate training at the UT Health Science Center in San Antonio with an emphasis in primary care.
